En af forklaringerne er, at der ofte mangler information om et farligt stof eller om den type arbejde, der kan for\u00e5rsage sygdom. Som f\u00f8lge heraf erkendes sammenh\u00e6ngen mellem sygdom og arbejde ofte f\u00f8rst, n\u00e5r en person er blevet syg: en reaktiv tilgang, der kaldes \u00bbsygdom-f\u00f8rst-tilgangen\u00ab.  <\/strong><\/p>\n<p>En bedre fremgangsm\u00e5de er at identificere farlige stoffer eller arbejdsopgaver, inden folk bliver syge: den s\u00e5kaldte \u00bbrisiko-f\u00f8rst-tilgang\u00ab. Det nederlandske nationale institut for folkesundhed og milj\u00f8 (RIVM) har udarbejdet en oversigt over metoder, der kan hj\u00e6lpe med at forudsige, om et farligt stof eller en bestemt arbejdsopgave sandsynligvis vil for\u00e5rsage en erhvervssygdom. <\/p>\n<h2>Vedvarende videnshuller<\/h2>\n<p>Der er stadig betydelige huller i viden om de skadelige virkninger af farlige stoffer og arbejdstagernes eksponering for disse. En af de v\u00e6sentligste \u00e5rsager er, at Risikovurderingen af de fleste kemikalier er baseret p\u00e5 test af oral toksicitet, mens arbejdstagerne hovedsageligt uds\u00e6ttes for kemikalierne gennem ind\u00e5nding eller hudkontakt. Derudover er der muligvis endnu ikke identificeret visse sundhedsm\u00e6ssige virkninger, og eksponeringsdataene kan mangle eller v\u00e6re undervurderet.  <\/p>\n<h2>Risikobaserede metoder: et overblik<\/h2>\n<p>Nye og fremkommende risici ved kemikalier (NERCs) kan p\u00e5vises i forskellige faser af et stofs udvikling ved hj\u00e6lp af b\u00e5de en \u00bbrisiko-f\u00f8rst\u00ab-tilgang og en \u00bbsygdom-f\u00f8rst\u00ab-tilgang. Den risikobaserede tilgang fokuserer p\u00e5 kemikalier og materialer, der stadig hovedsageligt befinder sig i innovationsfasen eller den tidlige markedsfase. Tegn p\u00e5 potentielle farer, b\u00e5de for kemisk sikkerhed og arbejdsmilj\u00f8, kan opfanges ved at kombinere data om farlige egenskaber med data om potentiel eksponering eller anvendelse.  <\/p>\n<p>Flere forskningsinstitutter og tilsynsmyndigheder har udviklet eller er i gang med at udvikle \u00bbrisk-first\u00ab-metoder til tidlig risikodetektion, herunder Den Europ\u00e6iske F\u00f8devaresikkerhedsautoritet (EFSA), Det Europ\u00e6iske Kemikalieagentur (ECHA), Det Europ\u00e6iske Arbejdsmilj\u00f8agentur (EU-OSHA) og RIVM. Yderligere metoder er opst\u00e5et gennem samarbejde og partnerskaber. RIVM har ogs\u00e5 identificeret andre tilgange, der kan fungere som risikobaserede metoder, s\u00e5som TICHNER-processen og flere in silico-metoder til tidlig p\u00e5visning af kemisk toksicitet.  <\/p>\n<p>Oversigten viser, at hver metode tjener et forskelligt form\u00e5l, og at b\u00e5de arbejdsomfanget og de n\u00f8dvendige oplysninger varierer betydeligt fra metode til metode.<\/p>\n<h2>P\u00e5 vej mod yderligere udvikling<\/h2>\n<p>RIVM har vurderet de identificerede \u00bbrisk-first\u00ab-metoder ud fra en r\u00e6kke kriterier for at fastsl\u00e5, om de er egnede til at styrke \u00bbrisk-first\u00ab-tilgangen. De mest lovende metoder er h\u00f8jkapacitetsmetoder, der tr\u00e6kker p\u00e5 store databaser og kombinerer data om fare og eksponering. Ingen enkelt metode opfylder dog alle kriterierne, og hver metode kr\u00e6ver stadig ekspertvurdering med henblik p\u00e5 prioritering og opf\u00f8lgning. P\u00e5 baggrund af denne vurdering blev to metoder udvalgt til videreudvikling: Lexces\u2019 in silico-forudsigelsesv\u00e6rkt\u00f8jer og RIVM\u2019s v\u00e6rkt\u00f8jskasse til hormonforstyrrende stoffer.
